Oliver, a 32-year-old male, presents to the emergency psychiatric unit with his husband. He is married with two children ages 1 and 4. He has an MBA and a successful career in commercial real estate. His chief complaint is stress and mood swings.
Oliver’s personal history includes growing up poor in a small rural town with a very cold and distant single mother. His parents divorced when he was 5 years old, and his father was never involved. He doesn’t have very fond memories of his mother and she hasn’t spoken to him since he married his husband. He describes his husband as loving, supportive, and a source of stability.
Interview with Oliver:
“I am really stressed, and my mood swings are all over the place. About a month ago I asked my husband for a divorce because of an affair that I had. It was only for a couple of weeks, but I couldn’t keep lying to him. At the time, everything seemed to be flying past me like a whirlwind… I launched a business with a new business partner, was making a lot of new investments, and the affair just sort of happened along with everything else. It felt like I was having a really lucky streak, like I was on top of the world! But in the last two weeks, everything has crashed. I realized my business part was a fraud and I lost my investment in the new business, to the tune of about $100,000. That money was for my kids college fund! I felt so guilty about everything; losing the money, getting conned, and of course, the affair and potentially breaking up my family. I can’t believe I’ve done this, and I’m so scared I’m going to lose everything. I have thought about suicide, but I know I could never do that to my kids. A few months ago, my primary care provider put me on escitalopram after diagnosing me with anxiety and depression. within a week, I felt amazing. Before that, I think my first episode of depression was after graduating college. It took me forever to find that first job and the stress of trying to keep my head above water was killing. To cope with the stress, I was drinking and doing other substances pretty heavily. As far as my family history goes, I don’t know a lot because I’m not that close with my mom, and my dad left when I was five, but I know she has a history of depression and anxiety. And I heard that my father was hospitalized for a psychiatric disorder, but I don’t know what the diagnosis was.”
