Week 7 Vignette
Sophia is a 15-year-old child who is referred to counseling by her school nurse because she keeps getting into physical altercations with other students. While in the nurse’s office for a scratch to her face Sophia disclosed that her parents plan to divorce. She confided in the nurse, (who shared with you) that she felt like the once stable ground beneath her seemed to have shifted and she is grappling with confusion, anger and a torrent of emotions. After signing the consent forms, her father tells you he is worried because Sophia stays in her room all the time and when she rarely emerges seems to be in a “foul mood” or irritable. He tells you that she has lots of scratches on her arms that she claims are from the fights at school. He says although Sophia’s mother recently left the home, they both share the same concerns.
It is evident Sophia is struggling beyond her immediate family crisis. She is struggling at school and has withdrawn from her extracurricular activities and her boyfriend. Sophia tells you that the external chaos seems to mirror an internal one, where various parts of her personality seem to battle for dominance. She also has feelings of distress when she thinks it’s her fault her parents are divorcing because she frequently argues with them about going out with friends, what clothes she is allowed to wear and how often she is allowed to spend time with her boyfriend.
