The Psychiatric History Tool is attached to provide direction on what information should be gathered during a psychiatric interview and assist you in determining what questions you would want to ask. Please review the rubric which will be attached.

2. What type of questions would you ask? Give 2 examples of how you would ask a question using this theoretical framework to gain the answers to your questions when completing a psychiatric evaluation? Be specific to the lifespan age you’ve been assigned.
3. Using this framework, what would be your focus for treatment?
4. What is the plan (with rationale) based on the theoretical framework? What is the reason you decided to take this approach with this patient?
5. Do you feel a different theoretical framework or approach would be better than the one you were assigned in treating this individual? Provide rationale.
6. Give a brief descriiption of your assigned screening tool. Explain its use, interpretation, and any important information related to the tool.
