Background: It is now time to implement your evidence-based, health outcome-focused educational project in accordance with your curriculum plan for the identified patient population. Before you begin this implementation, be sure to monitor constraints. These include being on BUDGET and on TIME when delivering the objectives of the project to your targeted patient population. You will develop an evaluation method to accomplish this. As you educate the patient population, be sure to address any CULTURAL or DIVERSITY issues you have identified. Clear, consistent, and coordinated communication is of utmost importance as you deliver your educational project to your identified patient population. Once you have completed the delivery of your educational project, you will begin your analysis of this implementation phase.

Purpose of Summative Assessment 2:

To develop an evaluation method for determining if the project is being implemented on time and within budget and is meeting original project objectives
To deliver or implement your evidence-based patient education curriculum plan and tool(s) developed in the Summative Assessment 1 to the identified patient population
To analyze the implementation phase of the evidence-based patient population education plan
Task 1: Develop a Method for Monitoring Your Progress Against Plan and Analyzing the Implementation Phase of the Educational Project with the Identified Patient Population.
Identify key measures that will help you monitor the project progress of the educational project against the plan you developed in NSG/516ED.

Create a list of constraints you may encounter as you implement your educational project. For each constraint, write 2 to 3 sentences explaining how you will address each constraint to mitigate them.

Identify educational objectives/outcomes to measure during implementation of the educational project with the patient population:

If they are not, how could you change the project on the fly to bring the project back on track to meet stated objectives and/or outcomes?

Create a 1-page assessment to measure the efficacy of your educational project.
After you deliver your educational curriculum plan and content to your patient population, have the patient population complete this assessment. This assessment will measure whether or not the patient population achieved the learning objectives you set forth in the curriculum plan. Collect the results of the assessment.

Task 2: Present or Deliver the Educational Curriculum Plan and Content to the Patient Population
Present or deliver the educational curriculum plan, tool(s), and content to your patient population. This includes your direct presentation and interaction with the identified patient population.

Write a 1-page summary of the process and tools you used in the delivery of the educational curriculum plan with the patient population.

Task 3: Analyze the Implementation Phase
Complete an analysis of the implementation phase of the evidence-based educational project to your patient population using both the list of constraints and assessment results, both of which you created in Task 1.

Write a 2-page summary analyzing the results of the implementation phase of your educational project.

Summative Assessment 2 Document
Outcome: The purpose of Summative Assessment 2 is to implement the educational project with the identified patient population. This assessment will also measure whether your project meets budget and time constraints. It also analyzes the implementation phase of the educational project.
