Health Behavior Change Final Project
Health Behavior Change Final Project – DUE ON 4/24 by 11:59 pm
One project = 70 total points possible
Late work will result in a 15 point deduction per day starting at midnight on the due date.
At the beginning of the term, students must choose a health behavior that they would like to adopt,
quit, or improve. Some examples of a healthy behavior change could be exercising more, eating less
junk food or fast food, eating more fruits and vegetables, cutting back on smoking/drinking, getting
more sleep, etc. You have a wide variety of behaviors from which to choose. You will complete the
“Health Behavior Change Contract” in the text and submit it to the instructor during the second
week of class. Over the course of the term, you must keep a weekly journal about your progress
towards changing this behavior and can use charts, pictures, etc. to keep track of your progress. The
journals are due each Friday beginning week 3 of the semester (see below for details). Ultimately,
you must write a 5-page paper describing your behavior change process. Please note that the 5
pages required for the paper refers to your paper content; as such, your reference page and title
page do not count toward the 5 pages. The paper should follow the format described below:
Title page; name, date, title of behavior change (does not count toward 5 page total)
Section 1 (2 pages): Current research on your health behavior (description, links to diseases,
prevalence, maintenance, benefits of changing the behavior, etc.). You must use at least 2
research articles from peer-reviewed journals of your 4 required sources. Use of all 4
references must be in this section of the paper. Heavy with citations in text throughout this
section and anywhere you are stating something as fact you must find that information in a
credible source and put it into your own words to back up what you have said then cite it.
Current research on your health behavior (description, links to diseases, prevalence,
maintenance, benefits of changing the behavior, etc.). You must use at least 2 research articles
from peer-reviewed journals.
Section 2 (2 pages):
1.Why did you choose this health behavior?
2.Which health behavior change theory did you use to address your behavior change?
3.Describe the health behavior change theory and why you chose it.
4.Discuss your process of moving through the different parts of the theory.
5.Did you make any progress with your behavior change over the duration of the class?
6.What was difficult about changing the behavior?
7.What strategies for change did you use?
8.What helped you in changing this behavior?
9.What are some long-term benefits of changing this behavior for you?
Section 3 (1 page): Provide a summary of your journal entries and thought/emotional process
you went through during the entire term. You may also include your tracking charts, pictures,
etc. in this section.
Final Page: References. Remember that your reference page does not count toward the total
required page number. You must include at least four total references. You must use APA
referencing 7th Edition.

Paper must be a minimum 5 page Microsoft Word document
Must be double-spaced
12-point Times New Roman font
1” margins all around
You must include at least four total references. All four references MUST be from peer-reviewed
journals or government supported websites like the CDC or NIH or ACSM, or SAMHSA (to name
a few). Students MUST reference within the text of their paper throughout. Use
