Case Study:
Dante Road Homecare is a for-profit home-health agency affiliated with Dante Road Hospital, a 55-bed rural facility. The agency is an early adopter of the homecare services that are now integrated into the plan of care for patients with chronic diseases. The tele-homecare technology integrates data from portable monitoring devices, including, digital weight scales, videophones, blood pressure cuffs, and spirometers.
After six years, the agency is beginning to see the benefits of investing in the tele-homecare infrastructure and find this system reduces costs and enhances the quality of delivered services. The monitoring allowed by this technology has reduced staff travel cost and time and has made scheduling homecare visits more efficient and given the ability to intervene proactively to prevent adverse events.
The organizational leaders are examining the possibility of integration of a PHR and a health social network site into the existing tele-homecare infrastructure. A positive feature of this social network is the ability for patients to access and manage their personal health information and to find and connect with others with similar chronic conditions for sharing and support, and for family and caregivers to participate in virtual support groups.
The technology company has held meetings with the Dante Road healthcare providers regarding this plan. Many of the homecare staff are supportive of the opportunities for information and communication sharing, but others have concerns.
