AA is a 36-year-old woman who presents to the
outpatient clinic for a follow-up.
CC: Insomnia.
HPI: Over the past two months, she
has been the responding police officer for numerous cases of child abuse.
She has distressing memories,
disturbing recurrent dreams and nightmares about the children she has helped.
She feels estranged from her
colleagues and has had difficulty concentrating at work. Recently, she has
started to dread going to work out of fear she will have to respond to another
case of abuse.
The patient is very distressed by
these symptoms and requests medication from you.
PMH: Denies
Current medications: Sertraline 25mg
daily.
OTC Benadryl 50mg PRN for sleep
-last used last night (started taking 2 weeks ago to help her sleep)
Substance Hx: 2-4 glasses of
wine every night -started 2 weeks ago to help her sleep.
What would
you recommend for treatment? Be specific in your answer and support your
