1. What payment and reimbursement systems could apply to the case (example: per diem) based on the care setting (outpatient and inpatient)?
2. To be fully reimbursed by the payment systems, what documentation would you expect to find in the complete medical record based on the face sheet ( consider medical necessity)?
3. In the inpatient and outpatient setting, how is the clinical data managed and who would be responsible for doing so? Include who can create the original data, where could the data be kept, who would review/audit the data and would maintain the data
4. Access the attached health record content & documentation for additional information that will assist you with completing this assignment

After answers to the questions are compiled, each team member must post a paragraph summarizing what they learned from this assignment about the concept of what clinical data is required for the payment and reimbursement system and why it is important to have documentation in the health record that supports the patient’s diagnosis and treatment.
