Using an assessment tool is similar to the nursing process in that it is a process that begins when the client is first assessed and continues throughout one’s care. Assessment tools assist the nurse to get to know the whole patient, including one’s thoughts, beliefs, and way of life. During the health assessment, observe and listen carefully to the client’s report. Display warmth, caring, and respect to encourage the patient to be open and share beliefs, concerns, and practices with you. Make sure to thank them for sharing their thoughts with you.

Select a client that meets the criteria of the assessment you selected. This person should be someone you do not know or do not know well. This assignment will help you practice and develop your patient/client interview techniques and skills, which is why this needs to be done with someone you do not know or do not know well. Identify yourself as a nursing student. Ask the client if they are willing to talk with you, and explain to the client that the questions need to be answered honestly. Obtain their permission to interview them. Persons younger than age 18 are considered minors, and must be under the care of a parent or guardian unless they are emancipated. Therefore, do not interview minors for this assignment.
Schedule an assessment interview with your client at least 2 weeks ahead. Give yourself enough time to complete the form and write your narrative/reflection.
Complete the associated assessment form with your client. Do not just ask questions and record answers mechanically. Conduct an interview using the methods and skills from Chapter 2: “Interviewing the Patient for a Health History” (pp. 11-21) in your Thompson’s textbook. Familiarize yourself with the content of the assessment before your interview so you can weave the questions into your conversation with the client. Be alert to cues that indicate you should explore an area in more depth.
Write/Document a summary of your assessment and your evaluation of the data. Use quotes from the patient to support your evaluation of the information.
Write your reflections, feelings, and thoughts about the interview. State some challenges you encountered if any. Also, identify 3 things you learned about yourself and 3 examples of verbal responses you gave during your interview.
