The paper should be organized into the following sections:
Introduction (Identification of the problem) with a clear presentation of the problem as well as the significance and a scholarly overview of the paper’s content. No heading is used for the Introduction per APA current edition.
Background and Significance of the disease, to include: Definition, description, signs and symptoms. Incidence and prevalence of statistics by state with a comparison to national statistics pertaining to the disease. If after a search of the library and scholarly data bases, you are unable to find statistics for your home state, or other states, consider this a gap in the data and state as much in the body of the paper. For instance, you may state something like, “After an exhausting search of the scholarly data bases, this writer is unable to locate incidence and/or prevalence data for the state of…” This indicates a gap in surveillance that will be included in the “Plan” section of this paper.
Surveillance and Reporting: Current surveillance methods and mandated reporting processes as related to the chronic health condition chosen should be specific.
Epidemiological Analysis: Conduct a descriptive epidemiology analysis of the health condition. Be sure to include all of the 5 W’s: What, Who, Where, When, Why. Use details associated with all of the W’s, such as the “Who” which should include an analysis of the determinants of health. Include costs (both financial and social) associated with the disease or problem.
Screening and Guidelines: Review how the disease is diagnosed and current national standards (guidelines). Pick one screening test (review Week 2 Discussion Board) and review its sensitivity, specificity, predictive value, and cost.
Plan: Integrating evidence, provide a plan of how a nurse practitioner will address this chronic health condition after graduation. Provide three specific interventions that are based on the evidence and include how you will measure outcomes (how will you know that the interventions have utility, are useful?) Note: Consider primary, secondary, and tertiary interventions as well as the integration of health policy advocacy efforts. It is okay to include high-level statistics in this section to clearly identify that there is a problem in your community. Also, be sure to introduce the community/geographic region that your paper focuses on.
Background of the disease including definition, description, signs and symptoms, and current incidence and/or prevalence statistics for your state and national statistics pertaining to the disease. (Include a table of incidence or prevalence rates comparing your state statistics to national statistics). You will find most of this information from online sources, such as CDC, Department of Health, local health department in your chosen state, Agency for Healthcare Research and Quality’s (AHRQ), American Cancer Society, and other national organizations that collect data. Please note – your table must be original and not simply a figure or table copied from another resource. Please refer to the APA manual for formatting guidelines for tables.
A review of current surveillance methods and any mandated reporting or methods for reporting the disease for providers. This section should discuss how data is collected at the state and national level. Who collects the data? What information is collected? Don’t forget about popular studies (e.g., Framingham Heart Study), CDC, and other national surveillance systems (e.g., Youth Risk Behavior Surveillance Systems/YRBSS). Please note – surveillance and screening are not the same thing. You will not be discussing how to screen for a specific disease in this section. Your focus is on how we collect data for the disease/condition of interest.
Conduct descriptive epidemiology analysis of the disease. Address the 5 W’s. Consider social determinants of health related to this disease. Include costs (both financial and social) associated with the disease or problem. You may find this data from recent studies, CDC, and other national/health organizations (e.g., American Heart Foundation)
Review how the disease is diagnosed, current national standards for screening or prevention, and pick one screening test and review its sensitivity, specificity, positive predictive value, cost and any current national guidelines or conducting which patients to conduct this test on. You must report the sensitivity, specificity, and PPV information for the screening test you have chosen. You may find this information in recent studies and in evidence-based screening guidelines (CDC, AHRQ, American Cancer Society, etc.). Don’t forget about the U.S. Preventive Service Task Force (USPSTF).
Provide a brief plan of how you will address this epidemiological disease in your practice once you are finished with school. Provide three actions you will take along with how you will measure outcomes of your actions. This section must be supported by recent, evidenced based articles (citations must be present). Each intervention must be supported by at least one recent, applicable evidenced based article per intervention. Don’t forget to discuss how you will implement your plan and how you will measure outcomes related to your plan of action(s). Remember, the three actions must be based on the needs in your chosen geographic area, as evidenced by the statistics you discuss earlier in the paper.
Conclude in a clear manner with a brief overview of key points of the entire disease. This section will summarize your paper, including major points from each of the above sections. Don’t forget the conclusion.
