This assignment will provide the student with the opportunity to analyze, use critical thinking
skills, and further evaluate a periodontally involved patient that the student has completed in
clinic. Each student will be required to submit a detailed case study on a patient who has AAP
classification of generalized moderate to severe periodontal disease with moderate to
severe inflammation and MCS level of 1A or higher. It must be a patient who was treated by
the student, the patient’s confidentiality must be maintained, and all personal patient
information must be de-identified. Instructions and rubric are below:
Instructions for Case Study Individual Assignment
The case study assignment must be submitted via word document on NYU Brightspace. Proper
grammar, syntax, and spelling are expected. The assignment must include a narrative portion
as well as a case study outline. The narrative portion must be 4-5 pages long and include at
least 4 scholarly references. All references must be in AMA citation. The case study outline
must include; radiographs, full periodontal charting and odontogram.
The narrative portion of your portfolio must include the below information:
Assessment: Summarize and discuss the examination performed, collect biological,
psychological, and social information needed to evaluate the medical and oral condition of the
patient. This includes the ability to recognize and manage behavioral factors which affect oral
health and to use that information to implement strategies that facilitate the provision of oral
health care.
Diagnosis: Discuss, analyze and interpret data to formulate a dental hygiene diagnosis related
to and congruent with the diagnosis of the dentist and other health professionals. Identify the
periodontal condition utilizing the American Academy of Periodontology Classification System
(AAP), including grade and stage.
Planning: Collaborate with the patient and other health professionals to formulate a
comprehensive dental hygiene care plan that is patient centered and based on current scientific
evidence. Identify various treatment modalities utilized in the management of the periodontal
patient based on the patient assessment findings and diagnosis. Make sure planned
interventions are evidence-based solutions, which will require citation.
Implementation: Discuss and summarize what specialized treatment was completed, include
preventive and therapeutic services designed to achieve and maintain oral health. Incorporate
any use of adjunctive treatment, such as chemotherapeutics. Include the amount of time for
each appointment and the specific care and oral hygiene recommendations completed at each
appointment. Explain the reasoning of the order of treatment and number of appointments
needed to completion.
Evaluation: Describe the results of the treatment. Include all findings from the re-evaluation
appointment and next steps in treatment to maintain periodontal health. If the re-evaluation
appointment was not able to be completed by the time of submission for this assignment,
include expected outcomes.
Self-Evaluation: Complete a self-evaluation and discuss the learning objectives achieved
through this case study assignment.
