The purpose of this assignment is to introduce you to the writing requirements in social work practice. Based upon all that you learned from your interviewee, you are to write a brief social history and process recording.
Paper length: 3-5 pages single spaced. No title or reference pg.
Must use Process recording format provided on Blackboard (download, type in your process recording).

· Goals: Based on your critical assessment, develop 2-3 goals. If this were a
real client you would work WITH the client to create goals but it is a practice
situation. Be realistic.
· Referral: Identify an actual formal or informal resource in the individual’s
community that you think would be beneficial to them based (e.g. an outpatient
substance use treatment program, or Gamblers Anonymous, self-self-group).
Provide the agency name, the individual expert, address, phone number and
type of help/services available.
Part II. Process Recording: Attach completed Process Recording (download sample
on Bb)– 50-70% of the columns filled out on each page as discussed in class.
