This activity is an GROUP ASSIGNMENT : Work with the members you signed up with for your Final Project.

a. Develop a SHORT case study; Introduce a patient with a certain diagnosis who has certain medical needs which you are helping discharge AS A CASE MANAGER from the hospital setting or clinic to the next level of care. The next level of care can be your choice i.e. home, assisted living, skilled nursing facility, rehab, nursing home. You have met with the patient and family to determine their discharge needs (shared decision making), and now will be meeting with other members of the medical team. Use the Shared Decision Making Model as your guide for engaging the pt/family
b. Discuss what information sharing you would provide/or questions you would ask of the following individuals tp prepare for discharge. This shows shared decision making as well as advocacy for the patient. (Be creative!)
In other words, what are you going to communicate with the following individuals to help develop a good plan for discharge? Think of what the pt/family needs are (which you creatively can make up).
Outline of who to communicate with:
i. the patient/the patient’s family, (think pt. engagement)- how would you ID what their needs are; include how you would assess for the patient/family’s literacy related to the information you are sharing. When including the Health Information you would be sharing, include some pt/family information websites and/or apps you would refer the patient/family to in your education. REMEMBER to assess for the digital or elder divide!
ii. the physician (what information do you need from the physician to help with discharge of this pt?)- think orders/referrals
iii. other medical staff and personnel (think interdisciplinary collaboration), Therapists, social worker, nurses have cared for the patient (what do you need to ask of these team members to help with planning discharge of this pt?)
iv. payor sources for equipment, therapies, medications etc (i.e.insurance companies)- what do you need to get ordered (and from who) and purchased/obtained to advocate for this patient to have a safe discharge
v. pt’s employer -if applicable
vi. an attorney- if applicable
