The goal of the Community Project is to have an opportunity to experience the role of the Community Health Nurse. For this project you will develop a 20 slide power point that involves three separate portions. 
A thorough community assessment. You will have an opportunity to complete a windshield and researched assessment on San Diego’s community. During the assessment you will provide a clear picture of the San Diego community. This will include:
San Diego Community Information (include location, type of community, geography & weather, major industries, primary employers, number of schools, universities, number and type of parks, type of grocery stores, restaurants (and types), produce markets, hospitals, clinics, public transporation).
Community Demographics (inculde: population, gender distribution, data for married/single, elderly, unemployment rate, mean income, number of households, types of housing)
Health Data. Include top three chronic diseases, top three infectious diseases for San Diego Community and healthy people 2030 goal for each. Include top three health concerns for San Diego community. Supporting data must be provided for each.
Social Data (include: community sports, recreation, religious organizations spiritual practies, volunteer organizations, support networks, community partnerships). 
A community teaching proposal focused on prevention. The research completed in the community assessment will identify prevalent chronic, infectious and priority community concerns. Create a teaching outline for the most prevalent chronic, infectious and priority community concerns. Each concern should have a minimum of 3 objectives, each objective should have a minimum of 2 topics that will assist in achieving the objective. Provide a proper educational strategy (verbal/written instruction, demonstration, role playing) on how you will present the prevention teaching. Provide a delivery method considering who the intended audience is. Lastly include an evaluation method on how you will ensure the teaching session was successful (survey, knowledge quiz, observation of behavior).
The last step is a disaster assessment of San Diego community. An opportunity to assess the readiness and resources that are available in case there is a disaster. Attached is a template for the disaster assessment to guide in completion of this step. 
The Community Project Assignment is a culmination of the  #1 Community Assessment, #2 Prevention Education Proposal and #3 Disaster Preparedness Assessment. All three steps must be included in the power point project. 
Develop a PowerPoint presentation with a minimum of 20 slides (not including the title slide or regerence slides
Place key point on the slide (maxiumum 8 words, 5 bullet point/lines. May include graphs and images. Graphs need to be properly cited.
Include detailed presenter (speaker) notes for each slide.
Full sentences and in-text citations using APA 7th edition are required.
Please ensure that when you submit PowerPoint presentations, you include a Word document (.docx) of the presenter’s notes and references.

Provides distinguishing, and accurate evidence base analyzing and integrating the main concept beyond the required (2) research sources with at least 1 source from a periodical database. Research sources are relevant, accurate, current (less than 5 years old) reliable, reflecting and validating the strength of the content.
Organization and Presentation
Ideas are organized logically, flows smoothly, and with an effective progression of thought from paragraph to paragraph connecting to the central position. Includes all required components (introduction, body, conclusion, citations, references, etc.). For a slide presentation, the design, font size, style, speaker notes/narration and spacing are optimal for audience viewing. Five or more of the following rules must be included. Slide presentation rules: 5×8 bullet points, Font ≥ 20 Notes associated with each slide. Narration (if required) is associated with each slide. Includes the required number of slides, excluding Title and Reference slides in the count. The presentation uses appropriate images or animation. (In-text citations are included). The slide presentation is appealing.
Writing and APA Formatting
Style and voice are appropriate to the given audience and purpose, but also show originality and creativity. Word choice is specific, purposeful, dynamic, and varied. Free of mechanical and typographical errors. A variety of sentence structures and effective figures of speech are used. Writer is clearly in command of standard, written, academic English. APA 7th Edition guidelines for formatting are consistently followed. Includes in-text citations and references.
