The final assignment should be a maximum of 7 pages total, not including the front page, references, or appendices. There is no set minimum, and the length of the proposal may vary depending on social problem, intervention plan etc. Proposals must be APA formatted, double-spaced, and free from typographical and grammatical errors in Microsoft Word format (.doc or .docx).
Front Page

Part I:  Identify Social Problem: (1-2 pages)
·      This will include the social problem for intervention. 
·      You should use secondary data (e.g., cross-sectional data, qualitative data, epidemiological data) and research findings to describe the problem.  
·      You should address the more than two of these questions to describe the social problem. 
o   What is the definition of the social problem?
o   How frequency does the problem occur?
o   How many people are affected?
o   Is this problem increasing?
o   How important do group members perceive the problem to be?
o   How important is the problem perceived to be by others?
o   Are there any negative impacts?
Part II: Intervention Plan and Logic Model (2 pages)
·      Logic Model: (1 page). The logic model includes goals, inputs, activities, outputs and outcomes using logic model table. 
·      Describe your logic model (1 page). This will include multi-level factors (risk and protective factors) which explains why you think the selected activities and/ or services will result in the desired outcomes, using research evidence to back up the claims. 
Part III:  Implementation (1 page)
·      Briefly describe the intended organizational home for the intervention, essential partnering organization(s).
·      Describe key factors to consider for the successful implementation of this intervention.
Part V: Evaluation (0.5-1 page)
·      Briefly describe how you evaluate the success of this intervention plan. 
References:  List references for any and all citations in APA style.
Appendices: Please provide any additional relevant materials, including (but not limited to): tables, figures, and/or survey instruments.
