Part 1 (300 words) 
Identify ONE physical, mental, behavioural or cognitive health and wellbeing issue from the scenario you have chosen.  Describe the issue. Briefly discuss the impact of the issue on the individual, with reference to literature.  
Part 2 (1,200 words) 
Identify the goal that your care plan will aim to achieve.  This goal must address the issue you identified in Part 1.  Explain the rationale behind the goal, and why this goal is of value to the individual.  Explain how achieving this goal could contribute to the individual’s recovery journey, with reference to recovery literature.   
Describe the intervention(s) you will use to achieve the care plan goal.  Explain the rationale for the intervention(s) you have chosen, and how these will contribute towards achieving the care plan goal.  Support your explanation with reference to relevant research, theory, and clinical guidelines. 
Your care plan must include discussion on how you communicate with the individual and their family/carer in order to develop a collaborative relationship that fosters engagement with the care plan and empowers the individual.
