Signature Assignment Writing Guidelines
Be mindful, this is a social work class. You will want to make connections to the profession of social work. Utilize the terms and theory you have learned in this class. Review modules 5 through 8 as they will help you with the signature assignment. Writing expectations include competency in the following: Formatting:
6-8 pages (excluding title page and references page)
Must be double-spaced with 1-inch margins and typed in 12-point Times New Roman.
Your essay should have a Title Page and References Page.
Your Signature Assignment should be proofread for spelling and grammar mistakes.
Other areas to pay attention to include:
Sentence Structure
Focus and organization throughout the paper. The paper should ow in a logical manner.
References and Citations:
You must cite all information used, including page numbers to avoid plagiarism
You should use a minimum of 2-3 sources from the TUW Library in support of your Case Assignment. Please note you can use other web-based sources in addition to the library sources.
Responsibility to “mock” client: You have an ethical responsibility to inform your ‘mock’ client that you are a BSW student, and this is a class assignment. Explain that it will take about 2 hours of their time. They do not have to answer any question that makes them uncomfortable. They may stop the interview at any time. You will use a pseudonym (not the real name).

Part 1: Biopsychosocial Assessment (Note: Must be completed BEFORE writing the Signature assignment)
For Part I of the Signature Assignment, you will complete a Biopsychosocial Assessment utilizing someone of your choice (friend or family). The intake conversation should take about 1-2 hours to complete, so please prepare your “client” in advance.
**If you do not have someone that you can utilize for your “client”, please contact me ASAP.
Locate the Biopsychosocial Assessment Tool found in the Presentation Section of Module 6 and Module 8 of this course and download it. You will utilize this form to document all the information you gather from your “client”. When all the information has been gathered, please save the document as a Word doc. You will submit this completed assessment form with Part II of the Signature Assignment.
Part II -Narrative Summary of Biopsychosocial Considerations: Written Signature Assignment
The paper will be start with an Introduction and end with a Conclusion. The signature assignment will be a 6-8 page essay. The following headings are required in your Signature Assignment. Respond to the questions under each heading in a narrative essay format. Refer to the background material in Module 6. In particular, the resource below will assist you with appropriate language for the narrative.

CLIENT SUMMARY
Identifying information – include information such as age, race, ethnicity, gender, marital status, children, employment. Presenting Problem – there may not be one with your mock client.
Biological factors: include information regarding physical functioning.
Psychological factors: include information such as coping capacities and aect.
Social factors: include information such as social roles, social functioning, social supports, nancial status.
ETHICAL DECISION-MAKING:
Assess from an ethical decision making prospective and explore your ethical duties as a social worker to your client as outlined by the NASW Code of Ethics. Please include specic standards to support your points.
Discuss any ethical dilemmas or considerations you need to assess (suicidality, Intimate Partner Violence, child or elder abuse, etc).
DIVERSITY, CULTURAL, AND SPIRITUAL ISSUES:
Discuss the role that diversity, culture, and spirituality play for your client based on the information gathered in the Biopsychosocial Intake Assessment. (Be sure to include any of the client’s personal characteristics, attitudes, language, communication style, beliefs, and roles you must take under consideration as you assist this family.)
Explore and discuss what role privilege and/or discrimination, marginalization, oppression, systemic racism, and inequalities have impacted their life.
