Sample Treatment Plan
For this assignment, you will create a sample case study, through which you will demonstrate application of various principles of applied behavior analysis in a practice setting of your choice. The hypothetical client you identify should demonstrate the need for a skills-based assessment to address the primary area of intervention, as opposed to an emphasis on problem behavior reduction. The case scenario will consist of the following components, and will use the provided treatment plan template:
Identify a hypothetical client, including setting for service delivery, age, and any diagnoses related to the need for behavior analytic services
Describe the reason for referral, including socially significant areas of intervention that you choose to address through your treatment delivery
Summarize hypothetical indirect assessment results, including an overview of the assessment(s) conducted and the findings related to implications for treatment planning
Summarize hypothetical direct assessment results, including an overview of the assessment(s) conducted and the findings related to implications for treatment planning
Recommended treatment dosage and schedule to include:
Risk-benefit analysis related to a client seeking services in this particular area of competence and your decision whether or not to begin treatment immediately
Explain reasons for and against commencing with service delivery upon receiving the intake referral and identifying the primary area of need
Hours, supervision and treatment schedule and locations
Five Skill Acquisition Goals based on the skill-based assessment results
Summarize (3) specific principles (e.g., conditioned reinforcers, high-probability instructional sequence, etc.) that you anticipate incorporating into the treatment, based on assessment.
Summarize a coordination of care plan to include the stakeholders involved, and coordination efforts to achieve treatment collaboration.
Summarize reasons for discharge that are and are not clinically indicated.
Integrate e ethical, legal, cultural, and social validity considerations related to providing services.
Identify specific standards set forth in the BACB Ethics Code for Behavior Analysts.
Discuss how one of the core principles in the Ethics Code for Behavior Analysts is upheld as a result of your decisions and actions
Summarize one Task List item from areas A through I that is relevant to your chosen assessment and intervention development process.
For each Task List item identified, briefly assess your own competence in and a plan for development (2-3 lines per Task List item discussed).
Your assignment should be a completed treatment plan template, with title and reference pages, and should include the following elements:
Title page: Provide your name, title of assignment, course and section number, and date.
Body: Answer all the questions in complete sentences and paragraphs, with in text citations for support.
Your responses should reflect professional writing standards, using proper tone and language. The writing and writing style should be correct and accurate, and reflect knowledge of skills and practice of the profession.
Reference page: Sources listed in current APA format.
Include a minimum of three scholarly or academic sources to support your responses and conclusions.
Use Arial or Times New Roman 12-point font, double-spaced and left aligned.
Use standard 1″ margins on all sides.
Use current APA formatting and citation style.
