Conduct a web search to locate an annual report for a public health department within the United States. If possible, select a local health department in your geographic region. Identify the funding streams reflected in the annual report (i.e.: fees, strategic plan monies, state funds, tax dollars, etc.) and succinctly report your findings (one to two sentences).
How does lack of consistency in public health service impact population health indicators in The Village? Note the public health indicators website in Resources. One or more of these indicators should be what you base this answer upon.
State one SMART goal for the at-risk population you chose from The Village, then write a priority community nursing diagnosis (4 parts) based upon the data in the case study.
Use the Nursing Process-create the following:
One at-risk population outcome objective
A minimum of two population-focused interventions to facilitate achievement of the stated aggregate objective
Add an evaluation statement of the stated at-risk population objective. An evaluation statement must be objective and measurable.
