Daniela was a lively and curious child who displayed an innate curiosity about the world around her. She enjoyed discovering new activities and had a strong passion for imaginative play. Daniela had a close relationship with her family and flourished in a nurturing and supportive environment. During childhood, Daniela consistently excelled academically and exhibited a strong enthusiasm for learning. She actively engaged in various school clubs and sports activities while also maintaining close friendships with her peers. Daniela had a positive outlook on life and welcomed new experiences with enthusiasm. Presently, Daniela is a 22-year-old college student who has been experiencing persistent low mood and other related symptoms for the past two years. She has been feeling sad and hopeless and has lost interest in things she once enjoyed. Along with these symptoms, Daniela has also been experiencing difficulty sleeping and a lack of energy. Over the past year, her parents and teachers have noticed that Daniela’s academic performance has declined, and she has become less engaged in her schoolwork and extracurricular activities.
Daniela experiences frequent feelings of sadness and emptiness and lacks interest and pleasure in most activities. She finds it hard to enjoy her hobbies, socialize with friends, or participate in school activities. Daniela often feels tired and lacks energy, which is causing her academic performance to decline. She also experiences difficulty concentrating, indecisiveness, and often doubts her abilities. Daniela’s well-being has been compromised lately, affecting her appetite, sleep pattern, and mental health. Her eating habits have changed, leading to overeating and weight gain. She often has an urge to indulge in comfort foods when feeling low. Furthermore, her sleep pattern is disrupted, making it challenging to fall asleep and causing frequent awakenings during the night. Daniela has expressed a sense of worthlessness and excessive guilt, blaming herself for her perceived failures and shortcomings. Daniela is currently experiencing significant symptoms that have significantly affected her daily life. She has become socially withdrawn, avoiding interactions with both friends and family. Her lack of interest in extracurricular activities, such as playing the piano and participating in sports – once a source of enjoyment and fulfillment – has added to her feelings of frustration and self-doubt. She may have found it challenging to accomplish her daily tasks and lost interest in activities that previously brought her joy. As Daniela grew older, she witnessed her mother’s ongoing battle with major depressive disorder. This could have had a significant impact on Daniela’s emotional well-being and development. Living with a parent who has a mental health condition can be challenging for a child’s emotional well-being and development, potentially affecting their sense of security, self-esteem, and overall mental health. Daniela’s mother’s depression may have influenced Daniela’s experiences and may be a contributing factor to her current symptoms. 
Daniela’s parents have scheduled an appointment with a mental health professional who specializes in working with adolescents to seek professional help. Based on the duration and nature of Daniela’s symptoms, she may be experiencing Persistent Depressive Disorder, commonly known as dysthymia. This condition is marked by a long-term low mood that lasts for a minimum of two years, with symptoms that are less intense but more consistent than those of major depressive disorder. Daniela expressed a desire to overcome her persistent low mood and related symptoms. She wants to overcome her social withdrawal and regain a sense of fulfillment and enjoyment in her life overall. The diagnostic impression the clinician came to is that Daniela has Persistent Depressive Disorder (Dysthymia) (300.4). Daniela needs a comprehensive assessment to gather information about her. Based on the evaluation, the clinician will develop a personalized treatment plan, which may include therapy, medication, and lifestyle changes.
