HLTH 2500 Final Project: Application of Health Behavior Theory or Model The purpose of this project is for you to demonstrate your ability to apply a health behavior theory or model to the development of strategies that promote behavior change in a specific population with a specific health issue. To successfully complete this project, you need to complete the following: 1. Apply one of the following theories or models to a specific population, health issue, and setting (choose only one theory or model to apply): a. Health Belief Model b. Theory of Reasoned Action /Theory of Planned Behavior c. Transtheoretical Model /Stages of Change d. Social Cognitive Theory e. Diffusion of Innovation f. Social Marketing 2. Develop two (2) health strategies using at least three (3) concepts/constructs of a health theory or model for each strategy. * *NOTE: The brochure you create in Week 4 is one of the two required strategies, so you only need to develop one additional strategy for this project. Your Final Project will consist of a PowerPoint presentation with speaker notes that act as a narration script for the slides. The speaker notes should be written as if they were going to be read word for word during the presentation while displaying the slides. It is not necessary to provide a recorded audio narration as part of this project. Tutorials on how to create a PowerPoint presentation and to add speaker notes can be found in the Walden Writing Center (https://academicguides.waldenu.edu/writingcenter). The presentation should be 8–9 slides in length (not including the title slide and reference slide). You should use at least two scholarly sources that are cited in the PowerPoint presentation and speaker notes and included on the reference slide. All intext citations and references must conform to APA guidelines. Your project should include the following slide sections: • SLIDE 1: Title Page o Include a title, your full name, the date, and the institution name. • SLIDES 2–3: Overview of the Population and Health Problem o Be sure to address the following in this portion of your presentation: 1. Identify a population of interest and a health issue that affects them. Research a population of interest (i.e., target population) and a health issue they face. The population of interest should be an identifiable group (e.g., college students, pregnant women, senior citizens, at-risk youth, etc.). Present the demographics of your population of interest (e.g., age, socioeconomic status, sex, race/ethnicity). Below is a link to a list of the leading health indicators for the U.S. You do not have to consider one of the health indicators in your choice of a health problem, but you are encouraged to review the Healthy People 2020 website. It includes a wealth of information available, including data sources on population issues and more. Identify a specific health behavior that is associated with the problem. For example, smoking (i.e., behavior) increases a person’s risk of developing lung cancer (i.e., health problem). 3. Identify the setting (i.e., school, community, worksite, or clinical) where the strategies you develop could be implemented. Explain why you selected this setting. o In the “speaker notes” section, include your presentation notes, which should be 2–3 paragraphs for each slide and should thoroughly explain the contents of each slide. SLIDES 4–7/8: Application of the Health Theory/Model o Using your identified health problem and your selected theory or model, explain the two health strategies you have identified/created to address a specific health behavior associated with your identified health problem. The purpose of this section is for you to apply your selected theory or model to two strategies for a specific health issue. o Strategy 1: Week 4 Brochure o The brochure that you created in Week 4 needs to be included in the presentation with any feedback from the Instructor and colleagues incorporated. o You are expected to apply at least two different concepts/constructs from your selected theory or model to each strategy. Explain in your presentation what constructs from your selected theory or model were used to promote or change a health behavior in the brochure. o In the “speaker notes” section, include your presentation notes, which should be 2–3 paragraphs for each slide. Your presentation notes should thoroughly describe the strategy you developed, how you used the theory or model, and how you applied the concepts/constructs to your strategy. o Strategy 2: In addition to the Week 4 brochure, your project must consist of the development of one of the following strategies: ▪ Series of print materials (at least two, such as flyers or posters) ▪ Audio PSA ▪ Series of Billboards (at least two) ▪ Fact Sheet ▪ Web page o You are expected to apply at least two different concepts/constructs from your selected theory or model to each strategy. Explain in your presentation what constructs from your selected theory or model were used to promote or change a health behavior in the brochure. NOTE: Your second strategy should be explained in the presentation. You do not have to create it like you did the brochure. o In the “speaker notes” section, include your presentation notes, which should be 2–3 paragraphs for each slide. Your presentation notes should thoroughly describe the strategy you developed, how you used the theory or model, and how you applied the concepts/constructs to your strategy. SLIDES 8–9: Project Summary o Provide a brief summary of your project. Describe any challenges you faced while developing it. Provide a brief reflection on how this application of theory or model to a health issue may help you in your career or advance your leadership skills. SLIDES 9–10: References o Provide at least two references in APA format (6th ed.) that support your presentation. NOTE: If you provide a reference, you must cite it in text at least once. Your citations must be in the PPT slide as well as in the speaker’s notes. The Final Project is due by Day 7 of Week 5. It must be submitted twice. First, you need to submit via the Final Project submission link in the “Week 5 Project” area. In addition, you need to submit via the “Discussion 5/6” area so your colleagues can review it as part of the Week 6 Discussion.
