affected by a variety of variables. SDOH are often difficult to address and
require equally complex, multilayered partnerships designed to correct their
trajectory by addressing the variables that underpin them. Clinical care alone
cannot ensure the optimal health of the client without addressing SDOH. For
this task, collect and analyze research about the SDOH impacting health,
well-being, or quality of life in your local community. Note: All references
used should be current, within the last five years.
SCENARIO:
You are employed as a health educator at the local health department in the Los
Angeles community. You are developing a presentation for Los Angeles community
partners on a social determinant of health (SDOH) affecting your community.
This presentation should describe the effects this SDOH is having on your
community and the need for implementing a new program within the community that
would address this SDOH. You are trying to gain support from other community
partners to help make this a success.

1. Provide three examples of how each of the five SDOH affects health, well-being, and quality of life using the attached “SDOH Template,” and include the template in your presentation.
B. Collect research about SDOH that affects health, well-being, and quality of life in your local community by doing the following:
1. Describe how you used data to identify three SDOH that are affecting Los Angeles.
a. Provide one source of data for each of the three SDOH identified in part C1, and include the sources on the reference slide of your presentation.
2. Discuss the effects of one SDOH affecting your local community. Include details on the specific population being affected and its overall impact on health, well-being, and quality of life for that population.
C. Propose a strategy to improve the health outcomes of a population impacted by specific health disparities by doing the following:
1. Describe a community-based strategy your partners should consider implementing to address the SDOH affecting the population identified in part B2.
2. Explain how health equity will be improved once the strategy from part C1 is implemented.
3. Discuss how you would resolve two potential risks of your strategy.
D. Acknowledge sources using in-text citations and references for content that is quoted, paraphrased, or summarized.
E. Demonstrate professional communication in the content and presentation of your submission.
