Part 1: Review the initial Treatment Plan submitted in Topic 4 specifically using a different color font to delineate from the previous treatment plan.
- Reassess your treatment plan diagnoses, goals, and objectives based on the new information provided.
- Fill out and submit a new treatment plan evidencing the changes made in treatment utilizing the treatment plan template provided.
Part 2: In a 1,000-1,250-word essay, answer the questions presented in a separate Word document, addressing the following:
- Examine the case and propose why the changes in the client’s presentation have occurred. Be sure to consider the theory to conceptualize these changes.
- Reassess the effectiveness and validity of the treatment plan, discussing how the client has met, made progress, or has not made progress toward treatment goals. Within your discussion, explore the obstacles that preventing any client success.

- Discuss how you would evaluate the resources available for you to make a referral for Lucy, how these referrals would be communicated to the client, and how you would collaborate with other behavioral health care professionals.
