Using the pdf i provide (course material), you need to construct your own hypothetical case study example of an individual with one of the disabilities discussed in the course.  
Your first task is to see the diagnosis you were given. You will then construct a psychological assessment report for your hypothetical client around the features of this diagnosis.
You will use the template for a psychological assessment report provided below. This template has been adapted from the sample psychological assessment report provided in unit four.
The diagnoses that you can use for your case study will be from the pdf unit 06:

and the dsm 5 pdf i have uploaded this is essential please do not use any external sources all the sources i provided must be used i will also link this one as a pdf although the link will help u cite it. 
You must draw on course material for this case study and you must cite the unit content from which you are drawing the information. If you are using the  pdfs without a unit number name on itand/or the videos/websites from the course content pdfs, please cite them according to the APA format. When drawing from the unit content I uploaded  please cite it as follows: (course content, unit insert unit number). For example, if you were using the content from unit four, you would cite it as follows: (course content, unit 4).

Psychological Assessment Report

Grade: Cite the appropriate grade that matches the age of your client
Reason for referral (3 marks):
a) What concerns led to this individual seeking a psychological assessment?
b) What questions are parents/the client hoping the assessment will answer?
Background information (5 marks):
a) Family information: Who does the individual live with? Any cultural or family factors that might be relevant should be stated here.
b) Development and early learning: How would you describe their early skill development (language, reading, social skills, etc.)?
c) Educational history: Report card grades, teacher observations, and any educational concerns?
You may find it helpful to review the ‘questions a psychologist might ask a parent during an intake for their child’ section in unit 4 for ideas about what information to include in this background information section.
At the end of this section, based on the information from across the course content (particularly unit 12), please provide an explanation as to why this information is relevant to a psychological assessment.
Document Review (2 marks):
a) What grades might be on your client’s report cards (A’s, B’s C’s, D’s – in particular subjects).
b) Include a summary of some possible comments that teachers might have made on your client’s report cards about their performance in school.
c) Might there be any other previous assessments completed for your client?  If so, describe them here.
Assessment Techniques and results (10 marks):
Consider the following list of skills that are often assessed during a psychological assessment (the list below is copied from the sample psychological assessment report from unit 4) and choose the skills that you think would be most appropriate to assess based on your client’s difficulties.
Once you have chosen the skills that you think are important to assess, state the following for each of the skills:
a) The name of the skill you want to assess along with the specific name of the assessment tool (this information is available throughout the course content).
b) Why this assessment tool would be appropriate to use to better understand your client’s challenges.
c) The potential score range for your client on each of the tests you chose (e.g. would they likely score below average, average, or above average given their diagnosis).

c) Using the course material from the unit 6 discuss two of the underlying skill deficits associated with that specific disorder.
d) Using the content from unit 11, describe one social-emotional/behavioural challenge or comorbid diagnosis that your client might experience and describe why these challenges might occur for your client specifically.
Recommendations (5  marks):
§  Using the course content, provide at least three recommendations and/or intervention approaches to support your client.
§  Using the content from unit 3, describe which tiered level of support would be most appropriate for your client and why.
