Provide approximately 2-pages outlining the pathophysiology of your client’s problem area. Eg. Sleep disturbance – describe the pathophysiology of the sleep cycle and what area your client is suffering in. Activity – describe what muscle groups are affected by your client’s inability to be active. Remember to cite this information. Cite your reference(s) using APA format. The summary should be written in your own words. DO NOT cut and paste from your reference(s). It is important to understand and discuss critical thinking related to your patient’s disease process or surgical needs.

PART 2 These are the instructions for the table below, the information that’s filled in the table is just a example to follow. The nursing diagnosis must be NANDA approved diagnosis!!!

Choose the 3 highest priority nursing diagnoses. Discuss 2 patient goals related to each nursing diagnosis. Nursing diagnoses must be NANDA approved and include all appropriate parts (stem, related to and as evidenced by). Patient goals must be Specific, Measurable, Achievable, Realistic and have a Time frame.

Describe 5 interventions for each of the three chosen nursing diagnoses. These interventions should improve patient well-being and include any patient/family teaching and nursing care for the patient and family. Interventions should take into consideration the patient’s personal health goals and interprofessional health care team collaboration necessary.

Patient Teaching should be described throughout your interventions. Should also include the patient response to teaching if applicable.

Evaluate your plan of care with a focus on patient goals. If timelines prove to not be realistic for your Care Plan, outline how you would evaluate whether your goals and interventions were met and any changes to goals or timelines.

Citations should be used throughout the assignment. Use your texts and at least 3 nursing professional journals.
