Part 1
· Describe your program in a few sentences, i.e., target audience and interventions or activities.
· State the questions(s) your evaluation is designed to address including questions to assess both process and outcome/impact of your program.
· Describe the proposed evaluation study design, i.e., what are you going to do to assess the outcome of the program; this includes identifying both your independent variables (various activities or interventions in your program) and your dependent variables (those outcomes you are hoping to impact).
Part 2 (Beginning with sampling frame and sampling strategy)
· Provide an overview of the methods(s) that you will use to evaluate the program (how you will collect the data and the timeframe for data collection).
· Describe the sampling strategy, including the sample frame that you will use to select the study population for the evaluation of your program.
· Identify the data elements required in the evaluation of your program, and describe the data source or how you will collect this data, i.e. data collection tools and sample measurable questions.
· Describe how you will analyze the evaluation data (i.e., what statistics will you use to assess the program) along with the timeframes or groups that will be compared.
· Identify study limitations, potential bias, and the generalizability to other populations; this is your internal and external validity.
