Many people have unauthorized access to opiates and tend to misuse it. We should be concerned because it leads to fatality.

How many people acquire or die from this health issue/disease annually?
What population(s) or specific group(s) seem to be more impacted by this health issue/disease? Compare and contrast their prevalence/incidence with the general population.
What is causing this health issue/disease in that particular community and/or with that particular group(s)?
What trends do you see in the data you reviewed? Has this health issue/disease been decreasing or increasing in the community?
What factors seem to be driving the increase/decrease of this health issue/disease? Is this increase/decrease a recent development or has disease always been transmitted/caused in this manner?

Identify and describe the health education and promotion model that will be used in your health interventions (e.g. Theory of Reasoned Action, Health Belief Model, Transtheoretical Model of Change, Social Cognitive Theory, DIffusion of Innovations, ect.)
Make sure you identify the main concept, constructs and variables of the theory you have selected.
Provide justification of why you chose to use this particular health education and promotion model.
What level of influence is this theory designed to impact (intrapersonal, interpersonal and/or community)?
Describe how this particular theory has been successfully used in other communities and/or to address this particular health issue?
Identify how the main concept and constructs of this theory address the main risk behaviors for this health issue/disease in your selected target population?
As a result of implementing this theory, what specific risk factors do you anticipate will decrease or stop? Are there any protective behaviors that you anticipate will increase? List what these are.
IMPLEMENTATION OF INTERVENTION
Describe how your proposed interventions will be implemented. This section should be written in sufficient detail so that if your team was not able to complete the project, someone else could pick up your plan and implement the same program.
For this reason, your implementation plan should answer: who will implement that intervention; when will intervention be implemented; where will intervention be implemented with what frequency will intervention be conducted; and how will participants be recruited and/or invited to participate.
This section, essentially communicates the how, when, and for how long, and by who of your health education and promotion program.
