My case is :
A 85 year old female consulted a general practitioner (GP) after completing one week antibiotic course due to upeer respiratory tract infection . She also complains of blurred vision on the right eye, back pain and nasal congestion.
The GP performed history taking and asked the patient to narrate the symptoms she was experiencing. The back pain started three months ago and now becomes severe also she find difficulty performing self care activities and work requirements and she worried as she lives alone.
When asking the patient about other health conditions she denies any other conditions but when thw dr asked her about the medication she took, she mentioned ventoline which indicated asthma and COPD. The
Regarding the surgical history the patient had done left eye cataract surgery,bowel resection, total hysterectomy and she was on chemotherapy 20 years ago as she was diagnosed by malignant melanoma.
Regarding this she mentioned that one of her friends told her that she will die soon as she suffers from multiple morbidities bit . However, the doctor showed empathy and reassured her that the timing of death is unpredictable and encouraged her not to worry.
The doctor started to examine the vital signs and start to auscultate the lung also a fundoscopic examination for the Right eye to reassure that everything good and makes the patient feel comfortable.
The doctor has prescribed an ocean spray for her nasal blockage and referred her to ophthalmologist for further evaluation of cortical cataract on the right eye and ordered XRay for back pain evaluation.

I would like mentioned as well that she is elderly patient with a lot of medication not only ventoline and she has dementia as she forget a lot of things and its polyphamacy we have to reduce her medication.
