Milestone 2: Original Contribution Change Project Paper
Due at the end of Week 6.
You have identified a quality care or patient safety issue which negatively impacts patient outcomes and have determined evidence-based practice strategies which could solve the issue. Equipped with that knowledge, you will now write a paper guiding implementation of activities to accomplish that goal. You will use the paper template and the information you gathered in the first milestone.
You will collaborate with key stakeholders (unit manager/other departments in the practice area as well as at least one interprofessional stakeholder) for the purpose of gaining a global perspective. You will use this data to support implementation of your plan within the clinical practice setting.
Step 1: Compose
Compose an 8 to 10-page, APA-formatted paper consisting of the following components:
Change Model Overview:
Overview of the Star Model of Knowledge Transformation©.
Define the scope of the evidence-based practice (EBP), including the PICOT.
Identify the diverse stakeholders and determine the responsibility of the team members and members of the interprofessional (IPE) team.
Evidence:
Conduct internal and external searches of evidence.
Integrate and summarize the Evidence Summary worksheet from Milestone 1.
Develop a recommendation for change.
Translation:
Develop an action plan for implementation within the clinical practice setting, including team members and responsibilities.
Include measurable outcomes for evaluation.
The plan will guide your implementation of the project within the clinical practice setting.
Include a reasonable timeline for implementation.
