As discussed in previous modules, patient/person-and family-centered care (PFCC) is a key element of safe, effective, quality care. Insurance payments are often linked to the provision of PFCC! As our health care system
has grown more complex and fragmented, it’s especially important that care is centered on the needs of patients and their families.
Is your facility/hospital/clinic currently providing PFCC? Are you aware of a mechanism to obtain input from patients and families? Do you know how the information is/could be used?
Reflect on the scenario on page 75 (Flagg, 2015) and its resolution on pages 83-84.
Think about a time you intended to provide PFCC but were not able to due to factors outside of your control. Did this situation have an impact on the provision of safe, effective, quality care? Briefly explain. (If you cannot think of a situation you experienced, think of a potential scenario where lack of PFCC could impact care.)
Review the PFCC care delivery models summarized in Box 6 on Flagg (2015), page 82. Choose one of these care delivery models and list 1-2 pros and cons (barriers) for implementation.
Next, briefly explain how using the Motivational Interviewing approach when helping patients change their health-related behaviors relates to at least one of the components of PFCC listed in the box above.
Finally, using the list of Quality and Safety Education for Nurses (QSEN) competencies, reflect on your own knowledge, attitudes, and skills (KSAs) as they relate to patient-centered care.
