Introduction
Treatment plans are part of direct practice in social work. After meeting with a family and completing an assessment, the social worker and family should agree on the family’s goals for treatment. The treatment plan identifies treatment goals and measurable steps to reach those goals, which are called objectives. There can be multiple objectives for each treatment goal. The social worker will also identify interventions that will be used to assist the family in reaching their goals. In practice settings, the family and social worker typically both sign the treatment plan. Treatment plans can be updated if diagnoses or presenting problems change during the course of treatment.
Assignment Instructions
For this assignment, you are to create a treatment plan for the Carl and His Parents (Word) case. Please review the Sample Treatment Plan (Word) to assist in completing the assignment.
Your treatment plan should include the following:
Identify one treatment goal for the family
Identify two treatment objectives
Identify two interventions based on the psychodynamic model, transgenerational model, or strategic model.
Note which theory each intervention is based on. The two interventions do not have to be from the same theory or model.
