Describe the SDOH model and describe contributing factors to your priority health issue.
Provide a summary of the one EBI (Primary/Secondary/Tertiary Level) to address the health priority.
This will become the focus of the program you have chosen to develop and implement for your paper.
Provide the level of recipient, stakeholders, and the role of PHN
Explore established policy resources such as: Centers for Disease Control & Prevention, the World Health Organization, Healthy People Objectives, the United States Preventive Services Task Force, and professional organizations (e.g., American Academy of Pediatrics, American College of Obstetricians and Gynecologists, American Cancer Society, ).
Note: You are not creating a health policy. You are to locate established health policy resources that can be applied and implemented with your target population and the identified health concern.
Policy & Program: Select and discuss an appropriate policy or program that addresses the population priority health issue
Discuss the general impact of health policy on health care
Provide a complete description of the health policy proposal
Clearly identify and briefly describe an established health policy (which will be the overall purpose of the intervention to be implemented) that is the focus of this paper.
Discuss how the policy will impact the county and address the population-specific priority health concern (Hint: consider the population diagnosis).
Describe in detail the first steps you will take toward presenting the policy and program to the community by discussing the following: (Include specific supporting data examples & citations.) Follow steps a-f in the template paper (see link below).
