Issue: Number of physical restraints utilized per month: 30
The Challenge: Develop a comprehensive QI plan to:
Reduce the NDNQI indicator: Implement evidence-based practices and interventions to directly address the chosen clinical issue.
Possible interventions: Increased family engagement to decrease the use of physical restraints.
Review all information on the PDCA page. Use the PDCA model to course correct the issue. Include the PDCA plans as a 1 page appendix.
Address the following questions in your paper supported by evidence from the literature.
What is the problem and why does it matter?
What is your plan?
What are the proposed interventions?
Who are stakeholders involved in the proposed interventions? Why are they selected and why are they important?
What data metric(s) will you use for evaluation?
What is your data collection method?
Why did you choose this data collection versus other data collection method?
Do you anticipate any unintended consequences (can be both positive and negative)? How might an improvement in a clinical indicator affect another?
Based on your data analysis, how can this intervention be implemented elsewhere?
Paper should be 2-4 pages (not including title page, references or appendix) and follows APA guidelines.
Minimum 5 references related to the selected indicator. The references should not be not more than 5 years old.
References could either Meta-Analyses, Systematic Reviews, Randomized Control Trials, Cohort/Case-control/Cross sectional studies, Evidence-Based Practice or Quality Improvement papers.
PDCA Cycle:
Define the specific problem you want to address within the chosen unit and its impact on both quality and non-clinical outcomes.
Set SMART goals for reducing the NDNQI indicator and improving outcomes.
Identify evidence-based interventions and strategies directly targeting the chosen problems.
Consider using the additional data points (HCAHPS scores, restraints, education/certification rates) to inform your interventions and potentially address multiple concerns simultaneously.
Develop a timeline for implementing the chosen interventions and monitoring progress.
For the NDNQI papers, complete the following:
Implement the planned interventions in the chosen unit, ensuring proper communication and training for staff.
Document the implementation process and any potential challenges encountered.
Check:
Collect data on the impact of the interventions on both the NDNQI clinical and non-clinical indicators.
Analyze the data to assess whether the interventions are achieving the desired goals.
Identify any unintended consequences or unforeseen factors affecting the outcomes.
Act:
Based on the data analysis, refine or adjust the interventions as needed to optimize their effectiveness.
Share the results of the QI project with relevant stakeholders within the unit and beyond.
Consider sustaining successful interventions and scaling them up to other units if applicable.
