Graded Final Summary of Knowledge Construction in the Practicum Experience this Semester:
For each course and individualized objective provide one example from your clinical practicum that demonstrates how you have grown in knowledge and competency in this objective (1 paragraph). This example may be drawn from any point in the clinical practicum.
In addition, for each objective where this applies, please summarize any additional experiences and learning that have occurred since your formative summary assignment. (1 paragraph). You may include a succinct reflection on the relationship of this learning to population health concepts and to specific course readings, discussions, and assignments. Your course instructor will have reference to earlier portfolio reports if you wish to refer to further details there.
Grading will focus on this final summary but will also take into account previous portfolio entries [please see Practicum Portfolio Rubric]. Please use the relevant course objective as a title heading for each short, 2 paragraph, section in your paper.

Completion of 45 clinical hours ( 5 of which may be field observation for Windshield Survey) and the signed Clinical Evaluation is a pre-requisite for grading of the Final Portfolio Summary and is required to pass and receive credit for this course.
