The factsheets can also be found in the Final Project module. These are not perfect examples, as some of the requirements listed below are not included; however, they should serve as excellent guides.
For this assignment, you will need to do the following:
Provide an overview of the health disparity in the target population you would like to focus on, including any changes in the incidence, prevalence, and mortality of the disease/behavior over time.
Use data from the National Health Interview Survey to gather statistics on the health disparity in your target population. You may also use this resource to gather information on risk factors and consequences of the disease (see numbers 3 and 4, below).
To gather this information, search for your health outcome/disease in the Keyword section of the table.
Choose 2016, or the most recent year available under the Filter Tables By section.
Choose the age group you’re interested in. Note: You may have to choose various groups until data appear in the bottom section of the table.
Once all information is complete, view the data using the link in the PDF or Excel section at the end of the table.
Identify several risk factors associated with the disease of focus (consequences of the disease, such as disability).
Describe several potential risk factors that produce the disparity for your target population (potential causes of the disease, such as poverty).
Use the Cliff Analogy, to describe the various levels of prevention that should be used to address the problem. In addition, describe how your target population could be moved away from the edge of the cliff.

Describe the mission and activities of a local or federal organization devoted to reducing or preventing the health disparity in your target population.
Cite ALL references as appropriate using a reference list at the end of your Factsheet. This includes any lecture content, readings, and all of the items you referenced for this assignment, including the National Health Interview Survey data.
