The purpose of this assignment is to create a detailed, personalized health plan that outlines goals related to three aspects of your health. Assessing health indicators and planning to nurture, develop, and maintain individual health is essential for optimal overall well-being. 
To Do:
Paragraph 1:
In this section, your primary goal is to introduce the topics you will discuss in your personal health plan. You must identify the three areas of your health that you want to research and develop a plan to improve or enhance. (Examples: stress reduction, nutrition, fitness, spiritual engagement, etc.)
Paragraphs 2-4:
In this section, your primary goal is to present research regarding each of your three areas of health (1 paragraph per health area). This is not a summary of the research you’ve found. Instead, this section should explain why it is important that you improve this area of your health. Here are some prompting questions to help you get started: 
According to current research, what are some of the known long-term consequences of poor health in your chosen area? 
According to current research, what are some effective strategies that individuals use to improve this area of health?
You must include a minimum of three scholarly research sources (in addition to the textbook and other course materials) in this section. 
3 SMART Goals:
In this section, your primary goal is to create an actionable personal health plan to improve your chosen three health areas. Your plan should be presented as three SMART goals (1 for each identified health area). Remember: A SMART goal is specific, measurable, achievable, relevant, and timely. If you are unsure how to format this section, I have included a formatting exemplar for you to review below. I recommend that you view it before you start creating your plan. 
Reminder: No weight-based goals. It is exceptionally challenging for people without the supervision of medical personnel to determine if they need to lose/gain body mass. Furthermore, it is even more difficult for people who need to lose/gain body mass to do so healthily and sustainably without professional support.
Paragraph 5:
In this section, your primary goal is to predict what might or will happen if you follow and achieve the goals that you’ve outlined. 
Here are some prompting questions to help you get started: 
If the goals you have identified are achieved, how would they impact your life? 
Do these potential outcomes make the effort required to meet the goals worth it? Why or why not?
Paragraph 6:
In this section, your primary goal is to reflect upon what you have learned about yourself through the creation of this plan and by completing your journal entries, coursework, and attending classes. This is NOT a ‘traditional’ conclusion paragraph.
Grading Notes: 
Please review the grading rubric, formatting exemplar, and the notes below and use them as a guide to structure and write your plan.
Your completed Personal Health Plan should be between 1750 – 2000 words and uploaded into D2L as a PDF or Word Document.
When creating a title page and reference page and citing sources in-text, please use APA7 formatting. For help with APA7 formatting, click HERE. The NLC Library also has tons of great resources. 
Research sources can include academic websites, peer-reviewed journal articles, or books. Your textbook and class resources are also often excellent resources to use, but they will not be counted as one of your three required research sources. The library and Google Scholar are always good ‘starting places’ to look for research material.
Good sentence structure, grammar, spelling (mechanics), etc., are a courtesy to the reader. While the occasional minor mistake is not a problem, please ensure that errors are kept to a minimum, as errors that impede the reader’s understanding of your thoughts will impact your grade.
