Use your lecture materials to determine three priority Social Determinants of Health (SDH) to assess for in the patient represented in the SOAP note. NOTE: Priority may refer to a strength in a SDH category necessary for the individual to attain/maintain health OR the priority may be a challenge in a SDH category identified as needing support or intervention for the individual to attain/maintain health.
In paragraph form construct a discussion that identifies those priority SDHs using data from the objective and subjective data sets where appropriate to support your discussion.

Module 4 Discussion
The social determinants of health are the surroundings in which persons are beginning life, exist, study, labor, act, love, and mature that shape an expansive array of wellbeing, operational and standard of living results and threats (Healthy People, 2020). Additionally, considerable qualities of their residence, the relationships of societal situations and an awareness of safety and happiness are also influenced by where people reside (Healthy People, 2020).
Within the provided SOAP note, the patient states that she does not go to the doctor. She is in a tough spot with a recent divorce, custody of her 2 small children and working at a retail store. It is unknown if there is the possibility of lack of transportation or her having the ability of taking time off to receive proper medical care. She is past due for some important medical wellness exams.
Sources that improve welfare can have a significant impact on public healthiness consequences (Healthy People, 2020). Instances of these means involve secure and reasonable shelter, admission to schooling, community security, accessibility to healthy diets, neighboring emergency/health facilities, and surroundings without dangerous contaminants (Healthy People, 2020). The patient must be having a difficult time regarding her current situation when she is using smoking excessively and alcohol use as her ways of coping. The patient has a poor prognosis and outcome with her current lifestyle, especially with including her family history of obesity, high blood pressure, breast cancer, heart disease and diabetes.
To establish a plan, she would need to be encouraged for the patient to act on the education, information and treatment options provided to her. The ball would ultimately be in her court so to speak. As her provider, I would want to make some sort of contact with her by phone or tele-health visit for follow-up. I would also want to involve social services or case management for other potential ways of providing support and to make sure she comes in for follow-up visits. I am hopeful that her visit to obtain medication for her headaches will lead to a healthier future for herself and her children that are dependent on her.

Social Determinants of Health
Health in humans is dynamic. It starts everywhere from homes, schools, communities, surroundings, and workplaces. We are personally responsible for our health. In addition, the economic, social, and other opportunities like resources, including adequate access to health care, availability of resources to meet our needs, environmental quality, determine the conditions of our health. They form the social determinants, which are conditions that are where people live, grow, age, and work (Artiga & HintoNn, 2018). The social determinants of health need to be integrated into programs that address health. The social determinants that need to be focused on are the creation of the physical and social environments that determine health. Social determinants vary depending on different groups of people, with the intensity of priority areas also varying. This paper will address three priority social determinants of health in a given patient.
The SOAP note gives insights into the situation of the patient. The patient does not go to check on the doctor. Her workplace is a small retail shop. Her relationship has ended, and she is dealing with a divorce. The divorce issue has her battling to meet the needs of her two small children. She does not mention her transportation means or gives information on access to health settings. However, her medical wellness exams are past overdue. She smokes often and drinks twice a week. The priority social determinants that are of the essence to the SOAP note on the patient include education, economic stability, and health and health care (Healthy People 2020).
Education has always played a fundamental role in health promotion (Healthy People 2020). Awareness plays a crucial role in making healthier choices in individuals. When people are literate about their health, they can make decisions about their health. The patients should be educated on the importance of cigarette cessation. A clear understanding of the long and short-term effects of smoking will help the patient to understand the health implications they have on them. She will be able to make the appropriate choice by stopping to smoke and making healthier choices. Also, she needs to be educated on the importance of minimal drinking as it affects her liver functioning and hence her blood pressure. Also, she needs education on the importance of self-care and stress reduction. The education will help her to initiate a plan to seek counseling to handle her divorce issue with minimal stress. Understanding how stress increases her susceptibility to infection will enable her to make plans to seek early interventions.
Economic stability is another key issue that needs to be addressed. The patient has to provide for her two children. The financial resources may not be adequate to meet her needs and those of her children. The needs include access to education, adequate housing that is safe and secure, access to adequate healthy food, health care access, and adequate transportation (Artiga & HintoNn, 2018). These form numerous social welfare issues that need to be improved. When improved, they have a significant impact in influencing the public health of the community. Welfare needs are portrayed in the life of the patient through her smoking activities. The susceptibility of the patient is increased through her lifestyle and genetic factors like family history of breast cancer, obesity, diabetes, high blood pressure, and heart diseases. A plan is needed to address the needs in consideration of the genetic and lifestyle to influence positive health outcomes.
Another priority determinant is health and health care (Healthy People 2020). The patient needs to be health literate and understand the impact of the medication on her. She also needs access to health care to get adequate treatment. She also needs to be consistent to get to her medical exams on time to assess her health situation. Health providers can reach out to connect her to the hospital, but she needs to be responsible for going for medications. The hospitals need to be equipped with adequate medications for the treatment to be available. Getting medications and medical exams on time influence the positive outcomes on health.
