Ethics Case 1
Paul is a 12-year-old male diabetic. He maintains his personal digital assistant (PDA), a hand-held device that interfaces with his glucometer and provides information based on inputted data from him and his parents. This information is transmitted to his MD/hospital, school nurse, case manager (CM), and to the parents’ home computer. All in an attempt to better control his diabetes. You are his CM. The PDA sends an update to you since Paul’s blood glucose levels have been rising steadily for the last 5 days. The physician has left a message for you to contact this family and have them come in for an evaluation. You know that both of his parents work so you send an e-mail message to the parents’ via their home computer asking them to bring Paul in for an assessment. Since you are in a hurry and will be seeing other patients when they return from work, you decide to add more information to the message than normal reviewing with them the importance of maintaining control over diabetes and expressing your concern since Paul has not checked in with you lately. You tell them that you think he might be over-doing it since he is trying to play football. You ask how they are doing and if they are still attending their counseling sessions. Somehow, Paul’s 4-year-old sister, Lilah, sent your email out to the diabetic list of contacts that both Paul and his parents interface with on a regular basis. The parents call everyone including you and are outraged that this information was sent everywhere….

Anna has hired twelve, 16-year-old girls from the local high school to help with document scanning. The scan jobs that they are doing typically consist of business documents. They take the documents, scan them in by hand, page by page, and then import them to CD and Film. When there is a paper problem, they copy the original and then discard it, using the copy. Anna has just been contracted by Low Mountain Health Center to scan patient records onto film and CDs. She is excited. The 100 boxes of medical records arrive within one week of signing the contract and Anna immediately puts them in the scanning storage to be done ASAP. The girls begin to work with these records without any further training.

Robert, a case manager (CM), has decided to create a list on the listserv from his own Internet service at home for his various patients’ illnesses. He establishes one list for each of the following chronic conditions: asthma, diabetes, and congestive heart failure (CHF). He automatically enrolls all of his patients that he has email addresses for into their appropriate list with the intention that they can share information and provide support among themselves, as well as, making things easier for him to share information with his patients. Everything is going well for the first month of the listserv exchange. However, a virus is introduced to the lists and everyone is contaminated. This virus has damaged the patients’ hard drives on their home computers and some patients’ work computers have been harmed. The patients contact Robert’s employer and demand to be reimbursed for all damages to their machines and are very upset that these lists were created in the first place.

Matt S. is the nurse manager of the Coronary Care Unit. He is logged on to his computer workstation to review and update patient records when one of his patients experiences a cardiac arrhythmia. At the sound of the alarm, Matt leaves his workstation and proceeds to the patient’s room to initiate treatment. Joe is in the hallway cleaning and replacing ceiling lights. When Joe sees the vacant workstation, he sits down at the terminal and searches for information about his neighbor who is a patient on another unit. Joe’s neighbor has been quite mysterious about his medical condition, and Joe is very curious about what is really wrong with his neighbor. Joe learns the truth about his neighbor’s health condition and shares this information with another neighbor.

Jill P. is a School Nurse in a suburban school district. Because of a crisis during her regular work hours, she is unprepared for a scoliosis screening and education program the following day. She decides to go into her office after hours to catch up. Her daughter Susan is working on a homework assignment and asks Jill if she can use the computer in the office because it has more sophisticated programs and a better printer than their home computer.
Jill agrees to let her daughter use the computer. While Jill is setting up for the screening, Susan accesses the student database and learns that four of her classmates were recently treated for sexually transmitted diseases. Susan shares this information in the lunchroom the following day. The news spreads rapidly through the school and one of the affected students learns that her “problem” is now common knowledge. She informs her parents who go to the school superintendent demanding to know how this could have happened.
