Written Assignments: Written assignments are “short papers”. Please utilize proper grammar and spelling. Your responses to these assignments need to be meaningful and reflect important information from the chapter, Power Point, lecture, or other sources. Be clear, concise, and thoughtful. Address all parts of the question, number each answer to the differ parts, and cite the guidance you are using in your response.
You must cite/reference your work. Cite where in the material (book, PowerPoint, lecture, or other source) you found the information you are using. An acceptable cite is (CM, pg. 256) (referring to Case management practice: Skills for the human services (4rd ed.), (CM PowerPoint, slide 21), or (MH, pg. 156) (referring to Mental Health Case Management-A Practical Guide (First ed.). Lecture (Gray, “week of lecture”, such as (Gray, Week 10).
Accuracy: Correctly addresses the question using information in the text, lectures, and supplemental sources. Cite where you found the information in your response by indicating the page in the text or other source when appropriate. Even when you are stating your opinion, you must reference the information covered to strengthen your position or you will not receive credit. Not citing work will result in a loss of 5 points.
Complete: Answers all parts of the question. Number responses according to the questions.
Quality: Written assignments are free of spelling and grammatical errors. When writing case notes do not use descriiptive metaphors. Instead describe the behavior in behavioral terms.
Week 2 written assignment:
You are a case manager for an agency who works with individuals with developmental disabilities. You have been assigned a case where your client (male) lives alone with his widowed mother. The mother has fallen and broken her hip and will be at the rehabilitation hospital for about 6 weeks. Your client cannot stay alone. He has a job at Goodwill Industries. County transportation takes him there every morning at 8:30 and brings him home at 5:00 p.m.
Additional Guidance: Think about what your client might need with his mother in the hospital. What things might she be doing that your client would need help with given she is not there. What services is he already receiving? Are there other needs the client may have that the mother or others may not be providing? As the case manager you are coordinating services from others based on need. You are not in most situations providing direct services. Think about services from other family members, friends, or agencies that might be available to meet those unmet needs. You will need to make some assumptions about the case but use logic in your approach. Let’s develop a tentative plan for your client.In your paper, address the following:
1. Discuss the steps you will take in this case (assessment, planning, linking, and monitoring). Clearly label each step and discuss the process you would take as the professional in this case. As an example; Assessment: (How are you going to assess needs? Use the text and lecture to support your approach)
2. Identify at least two services the client needs initially and one he could benefit from later. Answer why each of the three identified services are needed. NOTE: This should come from your assessment of needs and planning from question one.
3. How are you going to involve the client, his mother, and others (including other professionals and especially the family) in the process?
