All papers should include eight parts: 1) Introduction, 2) Description of Community, 3) Community Values and Practices, 4) Community Health Issues, 5) Social and Structural Determinants of Health, 6) Strengths-based Assessment of the Community; 7) Community Partners and Engagement Strategies; 8) Gaps and Primary and Secondary Data Needed; and 9) Conclusion.
The most successful papers use sub-headings to serve as signposts to guide the reader through each of the sections described in the outline below.
I.               Introduction: Provide a brief introduction to the community health needs assessment (~1 paragraph)
a.     Briefly introduce your paper topic (e.g., the focus of the community health needs assessment)
b.     At the end of the introduction, include an organizing sentence that outlines the primary objectives and sections of the paper.
II.              Part 1: Current context
a.     Description of community: Provide a revised description of the community you have chosen, including characteristics that define that community (e.g., identity, location, other unique characteristics), integrating feedback from application paper #1 (no more than 1 page)
b.     Community values and practices: Describe values and practices within the community within the existing social, economic, and political context (~1 page)
c.     Community health issues: Describe public health and well-being issues relevant to your chosen community (1-1.5 pages)
d.     Social and structural determinants of health: Describe social determinants of health as well as existing structural inequalities relevant to your chosen community (1-1.5 pages)
e.     Strengths-based assessment of the community: Describe existing assets and strengths within the community that are relevant to the community’s health (1 page)
III.            Part 2: Future directions
a.     Community partners and engagement strategies: Describe potential community partners with whom you might work as well as why they are important to partner with. Describe how you will engage with potential community partners, including gaining entry and building trust and trustworthiness with these groups or individuals. Make sure to discuss anticipated challenges with your proposed approach (1.5-2 pages)
b.     Gaps and primary and secondary data needed: Describe remaining gaps in knowledge and any additional primary and/or secondary data that would be needed to complete this assessment (0.5 page -1 page)
IV.            Conclusion: Briefly and clearly conclude the arguments made in the community health needs assessment (~1 paragraph)
