In adult patients discharged within 30-days from the acute care setting, does implementation of a comprehensive patient and caregiver education series, “Successfully Managing Your Chronic Condition”, compared to current processes, reduce 30-day all cause hospital readmissions within eight weeks?

 Developing and distributing educational materials to patients and caregivers.
 Offering training sessions for caregivers.
 Implementing a telehealth monitoring system to track patients’ health status.
 Establishing a care coordination team to oversee patient transitions from hospital to
