For Part 1 of your project, you will write a report about a real or fictitious patient’s illness, including the chief complaint (what they are there for), symptoms, any tests, the diagnosis, and the pathophysiology of the disease. Please note, the fictitious information that your created about the patient must be appropriate for the disorder in question.
This report must include the following information related to the real or fictitious case/patient:
Patient Initials – Do not provide your patient’s name because you will want to maintain patient confidentiality, but use his or her initials.
Patient Social History – Write a short paragraph to discuss your patient’s social history in terms of the following:
Family life
Occupation
Recreational aspects of their personal life, including exercise, smoking, drinking, drugs
Health History – Describe the health history of the patient in at least 200 words. Include the following information:
Diagnosis – Discuss the diagnosis of your patient in detail.
Patient’s chief complaint
History of present illness
Allergies
Medications they are taking
Past medical history
Past surgical history
Psychiatric history
Family history of illness
Patient’s diagnosis
Pathophysiology
Related laboratory tests/radiologic exams customarily used to diagnose and manage the disease/condition in question
The following should be included in this report:
An introduction of the paper – At least 100 words describing the content in the report. Include the diagnosis of the patient in the introduction
Provide the pathophysiology of the diagnosis in your own words (but include citations indicating where you read information).
Provide information about the laboratory tests (where applicable) that would have been used to confirm the diagnosis.
Provide information about radiological or other tests that would have used to confirm the diagnosis.
Provide a summary that discusses what you learned from researching and writing this paper (in at least 100 words).
Cite your sources and format your paper using the APA style guide.
Use a minimum of three credible sources. These sources can be from evidence-based articles, peer-reviewed journals, and/or textbooks (via the online library).
The paper should be no longer than 5 pages total (including title page and references).
