Throughout the course, you will practice skills that are important
when developing an effective public health intervention. Each week builds upon
the others and leads up to the final project, a mock grant proposal to an
imaginary funder: The Healthy Community Foundation.
The assignments guide you in developing the various components of
the proposal. For example, during Week 1, you will identify and describe a
priority population and in Week 2, you will conduct a community assessment
focusing on a priority health need aligned with a Healthy People 2030
objective. This assessment will form the basis of the Statement of Need for
your final project.
Keep in mind that the final project will require more than just
cutting and pasting the various assignments together. You should incorporate the feedback you receive
from your instructor and your classmates to improve each section. In some
instances, you will need to condense or expand upon the weekly assignment for
the final project. In the end, the final project should come together as a
thoroughly researched and well-planned proposal to fund a new public health
program.
Carefully review the scoring criteria below. These are similar to
the kinds of criteria a grant reviewer might use in a true grant selection
process. For the purposes of grading the assignment, the criteria align with
the rubric. It is important that you not only thoroughly answer the questions
in the Request for Proposals (RFP), but that you also consider the scoring
criteria.
Scoring Criteria
A panel of reviewers will score
the proposals using the following criteria:
● Need – To what extent does the proposal address a compelling need
that if addressed effectively, will advance health equity?
● Community Engagement – To what extent does the organization plan
to authentically engage the community in all aspects of the program planning
and evaluation process? To what extent is community engagement woven throughout
the proposal?
● Program Plan – How strong is the theoretical and evidence-based
foundation of the program? Are program goals and objectives appropriate and
realistic?
● Evaluation – To what extent will the evaluation plan measure
important outcomes of the program?
● Logic Model – To what extent does the logic model offer a cohesive
view of the inputs, outputs, outcomes, impact, and how those things relate to
each other?
● Budget – How well does the budget support the program plan, and
will the program be sustainable after funding ends?
Request
for Proposals
The Healthy Community Foundation seeks to advance health equity by
supporting evidence- based programs that meet the documented needs of
underserved communities. Nonprofit organizations may apply for up to $300,000
and can use the funds for a grant period up to three years.
You will refer to Healthy People 2030 to determine the health
behavior or condition to be focused on throughout the term. The following link
will take you to the Healthy People 2030 Objectives.
Proposals should be submitted as a Word document using 12-point
font, double spaced, and with 1-inch margins. The proposal should be written
for a lay audience who may not have formal public health training. Use language
that is commonly understood, avoid jargon, and explain any technical terms.
The proposal should include the following sections in the order
listed below. Please label each section and use proper AMA paper formatting. The numbers of pages for each
section are suggested, but not mandatory. However, the total proposal may not
exceed 2500-3000 words. Please address the following points and questions in

● How will you engage the community in program planning and
implementation?
● Provide a diagram and explanation of your effect theory. (Although
you included several interventions in your diagram for a previous assignment,
only include the intervention you are implementing for this proposal.
