Excluding COVID-19, Identify a communicable or infectious disease that meets the following criteria:
Affects the state where you practice nursing (New Jersey)
Is mandated to be reported to the state health department (A list of notifiable diseases should be available from your state health department’s website.)
That you are able to obtain recent data for (within the past 5 years)
Review the “Project # 1 Databases” within the “Project Resources” link under the COURSE CONTENT menu section
The worksheet should include data to address the three content categories and subcomponents: 1) Disease & Causality ; 2) Risk & Rates of Occurrence; and 3) Control & Prevention. Each category will include thorough and complete background descriptions, epidemiological data, and relevant analysis demonstrating the significance of the findings and implications for disease control and prevention. You may consider developing paragraphs of data for the sections to support the worksheet if you are out of room on the paper.

Provide current epidemiological data for the disease within your state including but not limited to morbidity and mortality
Morbidity (incidence, prevalence, disability, expenditures)
Mortality (case fatality rate and/or survival time)
Trend analysis (changes over time, across regions, etc.)
Prevention & Control
Procedures for reporting and surveillance
Interventions aligned with prevention levels (see table below)
Implement at least one mode of intervention to decrease the risk of transmission.
