During Unit 2/Module 5/Class 5, you will learn to use a web-based program (Qualtrics) for web-based survey design. You will be guided to create a survey in class that includes the elements below and submit the published link to your completed survey at the end of class. If you are unable to attend class or complete the assignment during class, you will have one week to complete and hand in the assignment on your own. Qualtrics provides help features and there are YouTube videos if you are having difficulty with any of the features. (5pts) 1. Open Qualtrics and begin a new, blank survey. 2. Add two blocks, so that there are a total of three blocks. 3. First block: a. Develop an introduction to the survey research with no response needed. “The purpose of this survey is to…By clicking on the “next” button, you consent to participate in the study. Please review the written consent that was included in the emailed invitation for a full version of the consent.” Use the Preview feature to review your work and ensure that there is no place on the intro page to add data. This would mean that you chose the wrong question type. Note: make sure this section is connected well with the scale that you will use to collect information in the second block. 4. Second block: a. Add a matrix-style question, with about 10 questions, and limited scrolling or distance from the response options. (For this part, you can select a scale and create an online survey for this. Or, you can assess the clinician’s attitude toward adopting EBP using the Evidence-Based Practice Attitude Scale (https://www.societyforimplementationresearchcollaboration.org/wp-content/uploads/2012/05/M_Aarons_EBPAS_20041.pdf) b. Use the Preview feature to review your work so far and decide whether you should have a page break. If you add a page break, make sure that it is clear to the participant that they’re still answering the same scale. 5. Third block: a. Add a Descriptive question that includes the following: These demographic questions will help us understand the characteristics of people responding to our survey. Please choose the answers that are the best fit for you. b. Add demographic questions (taken from the U.S. Census Bureau): i. A multiple-choice question with a single response: What sex were you assigned at birth on your original birth certificate? (Response options: Male, Female) ii. A multiple-choice question with a single response: Do you currently describe yourself as male, female, or transgender? (Response options: Male, Female, Transgender, None of these.) iii. A multiple-choice question with a single response: 1. Which of the following best represents how you think of yourself? (Response options: Gay or lesbian; Straight, that is not gay or lesbian; Bisexual; Something else; I don’t know.) iv. A multiple-choice question with multiple responses (selected from the U.S. Census): Which of the following describes your race (choose all that apply)? (Response options: White, Black or African American, American Indian or Alaska Native, Asian, Native Hawaiian or Other Pacific Islander, Some Other Race) v. A drop-down question (selected from the U.S. Census): 1. Which of the following best describes your ethnicity? (Response options: Hispanic or Latino, Not Hispanic or Latino.) c. Use the Preview feature to see how this looks and decide whether to insert a page break. 6. Add a progress bar. 7. Add a back button. 8. Publish the survey. 9. Distribute the web-based link to a published survey. After you have completed all steps, copy the link to a word document and submit that file. Include the name of the chosen instrument in your word document. Check and double-check that your submitted link works.
