Incivility, lateral violence, bullying and “clique behavior” are all terms indicating mistreatment from one individual or group against another individual or group. Bullying is a term often heard in grade school through high school. Some of you, especially if you work in the hospital/healthcare setting, may have hear the phrase, “Nurses eat their young.” What does that mean to you? Have you ever witnessed his type of behavior from nurses or other healthcare staff/providers?
Read Chapters 16 in your textbook as well the articles and TeamSTEPPS resources listed in the Module.
Case Study (true story): T.M., an pediatric nurse with 10 years of experience is looking forward to transitioning to the Labor & Delivery unit. The first day on the unit, T.M. was introduced to the rest of the L & D nurses and oriented to the unit. During her orientation she was assigned to be trained by the two most experienced nurses on the unit. On her second day on the unit, T.M., under the supervision of her preceptors, was caring for a primigravida patient (first pregnancy) in labor. The delivery of the baby was imminent when T.M.’s preceptors looked at her as they were walking out of the patient’s room and stated, “Good luck, you’re on your own!” T.M. had never had experience delivering a baby. She couldn’t call the charge nurse as one of these nurses was the charge nurse that day. And, she could not leave this soon to be mother due to the imminent delivery. She was able to page the obstetrician, and thankfully, everything turned out okay for both the mother and baby. T.M. realized her preceptorship was over. T.M reported the situation to her supervisor, but was told, “These are my two best nurses. They would never do anything like you just described!” T.M. realized she had a decision to make, she could either ‘stick it out’ or quit this unit and go work elsewhere. She decided to stay, and she for the next six years witnessed time and time again, what she dubbed the “A-team”, do the same thing to new and experienced nurses coming to work on this unit. T.M. would purposely volunteer to train the new employees, in hope that they would stay on the unit. Unfortunately, most did not. T.M. tried repeatedly to talk to the Nurse Manager and Director of the unit about the unsafe situations she was witnessing, but instead of investigating the situation, T.M. was labeled as the troublemaker. These two nurses would smirk and make mocking statements to T.M. when the Nurse Manager and Supervisor were not present. After six years, T.M. finally left this hospital, and considered leaving the nursing profession altogether.

5. What are the potential implications to the patient, in this case, both mother and baby, the unit, the hospital, and the profession of nursing?
6. As a future nurse, what can you do to mitigate bullying, clique behavior (group bullying), incivility, lateral/horizontal violence (any/all) behaviors?
