CASE STUDY: NDIS Name: Serena Waldorf Age: 37 Serena Waldorf is a 37 year-old married female of Lebanese ethnicity. She has a primary diagnosis of schizophrenia and has been an in-patient of a mental health hospital for 3 months. She had a deterioration of mental state due to non-compliance of medications. When unwell she became isolative and withdrawn, unable to manage her ADLs and routine. Prior to hospital admission she was living with her husband, young daughter and was working on a part time basis in a bank. During her admission she was seen by the multidisciplinary team and assessed by the Psychiatrist, Occupational Therapist and Diversional Therapist. Serena and the team identified the following goals: medication compliance, cooking for her family, household chores, budgeting, routine establishment and engagement in her leisure interests of beauty and exercise. The multidisciplinary team believe Serena’s mental state is now stable and are preparing a discharge plan. She will be returning to her marital home with her husband and daughter along with recommencing part time work. To support her in the community she was approved for an NDIS plan along with a community mental health team to support her during and post discharge. CASE STUDY: AGED Name: Nicholas Connell Age: 68 Nicholas Connell is a 68 year-old male living with his 65 year-old wife Lina Connell and pet dog in their own home. Nicholas was recently diagnosed with Parkinson’s disease. His symptoms include difficulty with walking, difficulty swallowing, loss of speech and reduction of motor skills, especially when holding cutlery. He is a high falls risk and aspiration risk. His wife Lina finds it difficult to manage his needs. Nicholas and Lina used to be very active, walking their pet dog daily, meeting up with friends at the local church and attending bingo at the RSL club weekly. His GP has referred him to the ACAT team to ensure his needs are supported in his home environment. Nicholas and the team outlined the following goals: improve mobility and motor skills with assistive technology and home modifications, walking his pet dog, increase socialisation/communication by reconnecting with his church group and bingo nights.
