Care Coordination
Nick is seven years old. He has been demonstrating inappropriate behaviors in the homeroom class at school. At times he is not focused. He hums and makes other noises when he should be working on his homework. He disturbs other students by kicking the legs of their chairs. When the instructor tells him to stop, he does but then he starts back up. Sometimes he ends up in the principal’s office for a time out. He has been referred to the school Special Educational Needs Coordinator (SENCO), who is acting as his key worker and has arranged a face-to-face meeting with Nick’s parents. Nick participates in an after-school club three times a week, and for the two other days he is picked up by his child care worker who provides afterschool care services. The SENCO worker, acting as the central figure in his care, arranges and keeps records of all the meetings. She is the primary point of contact for the team involved in supporting Nick.
