For this assignment, read the American Society of Health System Pharmacists (ASHP) Guidelines on Preventing Medication Errors in Hospitals.
In 2-3 paragraphs, discuss interventions that are used in your current or previous practices to prevent medication errors. Include in this discussion which interventions do you believe are highly effective and others that although in theory are highly effective are more challenging. For example, administering medication with no distractions- although this is very important at times this is very difficult to achieve on a busy unit.
