Discuss a transition plan involving the development of various life skills necessary to support successful adult living for students with intellectual disabilities after graduation from high school. Ensure to describe the life skills to be addressed, why these skills are needed, and how the development of these skills will be supported (i.e., who will provide support, where will these skills be supported, how often these skills will be supported, how will these skills be evaluated?).
